Upon arrival, please inform your server or manager of your chosen Party Package. To ensure a smooth experience, at least one member of your party should arrive within 15 minutes of your reservation. Late arrivals may result in releasing the table. No refunds will be given. You may bring freestanding or floating decorations, but confetti, glitter, and similar items are prohibited and may incur cleanup fees. Pricing excludes tax and gratuity. Per Maricopa County Health Codes, outside food and drinks are prohibited. A $100 deposit is required to secure your reservation, non-refundable and applied to your final bill. Deposits are non-refundable. The Forum may cancel reservations for event buyouts, with full deposit refunds processed.

First time visiting and I like the vibe in the place. Friendly staff and great music. Being a food critic I love sneaking in on these places and being blown away by the food. For bar food, they did not disappoint A+

Customer service is always great. Wait staff are excellent and very attentive. Food was good. The band, Slingshot, was extremely loud for that venue. We had to move tables to sit away from the front of the stage. We have been previously with other bands and there was not a problem. Only could stay for 1 set. The band even made a joke when we moved that they are a "loud band". No enjoyable on the ears. We will go back to the Forum again, just not with that band.

The servers were excellent - let me get that out the way first! They were so attentive and quick, very genuine. The food was good and came out hot!! Only issue I had was with the hostess and how they take and seat guests with reservations. Basically she told me if I would have come with a larger party, I would have gotten better seating. All the parties of 2 were seated way out away from the stage, even though I made a reservation weeks prior. Larger parties with no reservations were seated in more favorable tables! I don’t care for that process and I’ll think twice before we go there for another date night!

Nice club, wasn’t too busy this time surprisingly since the band was energetic and enjoyable even though they played songs to a younger generation then myself. I enjoy the classy atmosphere

Great time. Everything was wonderful. Coming back next time Splash is there. But I don’t like the high top table. Next time I’ll request a regular table.

The good was really good and the vibe was fun. But it was loud, due to band, so not a place for having table conversation… but fun to be entertained. We will def go back!

My experience was awesome! The service on the night I went was very slow. I had a hard time checking out. But I the whole group I was with which was 9 pl! Had a awesome time!the good was hot when it came out and the service was friendly!
